  396-1 Air Speed Check Zone Request Form  .doc Used by ODOT Districts when requesting a new or revised Air Speed Check Zones. The same form is used to abolish such zones. See TEM Section 301-10 for further information.
Publications / Standards
  196-1 Change of Address Form   For those who have a copy of the OMUTCD, this form can be used to advise us of a change of address.
  196-2 Revision Proposal Form   This form is intended for use in proposing revisions to any of the OTE publications.
  196-3 OTE Publication Order Form   This is an order form for use by local jurisdictions and public agencies.
  196-4 Address Verification Form   This form is included in the front of OMUTCDs as they are distributed, and should be completed and returned to the Standards Section to document receipt of the manual and to have your contact information added to the distribution list for that manual.
