Addition to Literature Review Requirements:

1. Determination of Eligibility file:

The list of properties determined eligible by the ACHP, despite property owner objection, is found in the NRHP notebook at the OHPO (listed at the end of each county). The properties are identified by DO (owner objection) on the NPS data base.

2. Transmittal of original OHI and OAI forms:

The final OHI and OAI forms are to submitted to ODOT with the survey report. OES will forward the original forms with the document to OHPO.
