How long does it take to become prequalified?

ODOT has 30 days to work the application upon receipt of a complete application.

If a company requests to be prequalified for multiple work types does a Work Type Request & Reference Form have to be filled out for each work type?

Yes.

Does the CPA have to submit Financial Statements along with complete financial footnote disclosures with the Prequalification Application?

Yes, the CPA must sign off on the application as well as submit a financial statement.
 

Will there be any changes in the Prequalification Application?

Yes, the Application has been revised to reflect the recent changes to the Prequalification Administrative Rules.  The new Qualification Application is now available for download.  For new applicants download the Prequalifcation Packet - New Applicants and for expiring contractors download the Prequalification Packet - Renewal.

Must all subcontractors be prequalified?

All subcontractors shall obtain a certificate of qualification, except subcontractors who will perform less than $500,000 of ODOT subcontract work in the calendar year and subcontractors who will perform work for which there is no corresponding work type. Prime contractors may employ subcontractors who do not hold a certificate of qualification, if the subcontractors' collective compensation does not exceed 15%, not to exceed $2,000,000, for a specific project. Annual limitations of $500,000 per subcontractor may not be exceeded.


What is meant by Key Construction Personnel?

Key construction personnel are on site persons in responsible charge of the work, having direct supervision of the contractor's workforce and subcontractors.

Do I have to submit Work Type Request & Reference Forms if I use the affidavit?

No.  Only when requesting additional work types.
