Yes, the DMA form is part of the Aircraft Registration Application and must be submitted each time an aircraft is registered under one the categories requiring payment.

What if the number of seats listed on my aircraft registration application (for renewal) is not the same as the actual number of seats in my aircraft?

The registration fee for your aircraft is based on the maximum certificated seating capacity of that aircraft. The FAA provides this information to the Office of Aviation. What if my Aircraft Registration Application is returned?

Here are some of the more common reasons an application cannot be processed:

DMA form - missing, incomplete, or not readable.

A new DMA form must be submitted annually as part of your paid aircraft registration application. No DMA form is required for aircraft being registered as; Sold, Not based in Ohio, or Not Airworthy.

Payment - check not enclosed, amount not correct, check not signed.

If the registration category requires payment, the check amount must match the amount shown on the Aircraft Registration Renewal form and be made payable to the Treasurer State of Ohio. There is no fee for aircraft being registered as; Sold, Aircraft Not Based in Ohio, or Not Airworthy.

Registration Category - proper "Category" box not checked.

Aircraft owner must indicate the registration category by checking the appropriate box. (New, Renewal, Sold, Aircraft Not Based in Ohio, Not Airworthy), and include the DMA form and payment if required.
